Steps to participate in the Neighborhood Quality of Life Study (NQLS)
Thank you for your interest in participating in the Neighborhood Quality of Life Study! Following are the steps that need to be completed in order to become a participant:


    Step 1. Meet study requirements.
    • Are you between the age of 20 to 65 years old?
    • Do you live in a house, apartment, or condo?
    • Are you able to walk?
    • Are you able to read or have someone read to you study instructions in English?
    • Do you live in a neighborhood that we are currently studying? If you received a letter from us in the mail, you are in our study area. If you did not receive a letter from us, click here to submit your contact information and a staff member will contact you within a few days to let you know whether or not you are in our study area.

      • You can contact a NQLS Research Assistant (toll-free) at 1-800-990-NQLS (6757) at any time to determine your eligibility or to ask any questions about participation.

    Step 2. Read and sign the Consent To Act As A Research Subject form.
      Read, print, and return the signed consent form to:

    Step 3. Once a signed consent form is received, you will be contacted by a staff member. A few minor details regarding participation need to be discussed.
      1. The staff member will answer any questions you may have about the study.
      2. You will be asked a few questions needed for our study, including which version of the survey you would prefer.
      3. A few weeks later, we will send you an Activity Meter.

    Step 4. Complete Survey
      Once you receive an Activity Meter, you will be contacted to complete a survey. This survey can be completed on-line, by mail, or over the phone. If you request the on-line survey, you will be given a internet address and detailed instructions. If you request the written survey we will send you the survey in the mail.
